Membership Program Terms & Conditions
Membership Eligibility
- Membership is open to individuals who are healthy and do not have health conditions which would exclude them from participating or receiving our services.
- Membership is non-transferable.
Membership Benefits
- Members are entitled to the benefits and privileges of the membership level they have enrolled in.
- The benefits may include, but are not limited to, access to specific services, discounts, or special promotions.
Membership Fees and Payment
- Membership fees are outlined at the time of enrollment.
- Payment will be processed according to the terms and frequency specified.
- Failure to make timely payments may result in the suspension or termination of membership privileges.
- All membership payments are non-refundable.
- By enrolling you agree to allow Hydration Sensation Wellness & Beauty to enroll you in automatic payment using the card you specify for membership fees.
Membership Duration
- The duration of membership is specified at the time of enrollment.
- Renewal may be automatic unless the Member provides written notice of termination as described in this agreement. This can be via email, text or in writing.
Termination
- Members may terminate their membership with a written notice via mail to 420 W. College Street Suite 104 Springfield, MO 65806, via text to 417-812-5575 or email at Sarah.Hydrationsensation@gmail.com
- The organization reserves the right to terminate membership for breach of this agreement, including disruptive behavior, non-payment, or any other violation of membership policies.
Changes to Membership Terms
- Hydration Sensation Wellness & Beauty reserves the right to modify the terms and conditions of the membership program at any time.
- Members will be notified of any changes in writing, electronically, or via our official website.
Liability Waiver
- Members acknowledge that certain services may carry inherent risks.
- Members release Hydration Sensation Wellness & Beauty and its staff from any liability resulting from the provision of services.
Privacy and Data Usage
- Hydration Sensation Wellness & Beauty will collect and use personal information as described in our Privacy Policy.
Governing Law
- This Agreement shall be governed by and construed in accordance with the laws of The State of Missouri.
